OncoForm | MIPS Considerations

When determining which structured data fields to keep or remove during your OncoForm build, keep MIPS in mind. Any MIPS-required fields should be kept in the AI-enabled template. See below for all requirements.

-Some measures require certain diagnosis codes (ICD-10)

Ongoing Treatment Events

-If your visit note template includes this section, adding hospice as a structured entry is important for many measures.

-Additionally, some measures look at the structured addition of a SNOMED code in this section.

Allergies

-The Allergy reconciliation workflow is required (but can also be completed from Summary page).

Structured documentation of allergies is important for allowing patients to be excluded from measures.

-Note: Allergies can only be viewed from the visit note and can’t be added or edited.

Medications

-Reconciliation workflow is required (but can also be completed from Summary page).

-Structured documentation of medications is important across many measures.

Tobacco Use and Cessation Counseling

-This section should sync with the Summary page for practices who have it set-up correctly. If it isn’t, then we look at the datanames here to confirm the patient’s smoking status and whether or not they were provided smoking cessation intervention

Pain

-Grading Scale score

-Treatment recommendation

-Pain grading scale is used for MIPS measure.

-Pain treatment recommendation (checkboxes) is also used.

Depression section

-Grading Scale score

-Treatment recommendation

-Patient is excluded from
depression screening for
the following reason

-Depression grading scale is used.

-Depression treatment recommendation (checkboxes) is also used

-“Patient is excluded from depression screening for the following reason” is important for measure exclusion purposes.

Performance Status

-ECOG grading scale

-Karnofsky grading scale

-ECOG + Karnofsky grading scale values are used for QM450 (as a denominator exception).

BMI/BP Screening & Plan

-QM497 requires checkboxes with specific datanames in order to complete the workflow of the measure. Most practices maintain a standalone OncoForm (MIPS 2024 [BP/BMI]) to track and fulfill this requirement. However, if your practice has a BMI/BP Screening & Plan in your visit notes, you must maintain this structured data field.
